In order to do business as a Sub-Agent through ASAIB you need to meet certain requirements:

  • Hold the proper state insurance license required to transact business (Copy required)
  • Have at least $500,000 per claim / $1,000,000 aggregate of Errors and Omissions coverage in effect with an approved carrier (Copy required)
  • At the time of your first submission only you need to send us a copy of your current insurance agent's license and a copy of the declarations page of your present E&O policy.
  • Complete one of our Producer Questionnaires.
  • Print, Sign and return 2 of our Producer Agreements.

To obtain a proposal:

·         Submit applicable ACORD application.

·         Submit copies of currently valued loss runs covering the last 3 policy years.  Provide detail on any large claims.

·         Complete the applicable forms below:

o        Elite Lodging Workers' Compensation Program

§         Complete a Supplemental information form.

o        Pacific Hospitality Program

§         Program Questionnaire (required)

§         Restaurant Questionnaire (if applicable)

§         Non-Owned Automobile Supplemental (if applicable)

§         Statement of Values (required for each location when coverage blanketed)

§         Building Checklist (required on buildings 20 years or older)

o        Employment Practices Liability Coverage Application

§         Application

o        Lodging Employee Benefits Program

§         Quote Request

§         Health Questionnaire
