For Youth Camp Programs, including Youth Athletic Camps (excluding football camps) for participants 18 and under, please use this UC Statewide Campers Audit Form to report athletic and other camp activities to be covered under the policy.

Please note that enrollment in this insurance program is intended specifically for the purpose of registered participants in Youth Camp Programs, including Youth Athletic Camps (excluding football camps), 18 and under.

If you should need accident coverages for any other type of participant activities i.e. non-High School, College/Undergraduate, Graduate and Post Graduate/ Doctoral Students please contact us directly. These other insurance programs may be underwritten to cover school-sponsored activities such as research, preceptorships, workshops, seminars, internships but excluding medical resident internships etc.

Instructions: Begin by entering the information for one camp activity, and identify whether it is an Athletic or Other activity. Click the Calculate Premium Due button to calculate the premium automatically. Please note that premiums are different for athletic and non-athletic activities. Once the form is completed, click the "Submit Form" button and the information will be sent to us.

We will provide you with an invoice by mail for your total campers insurance.
